If users require any support during the process, scroll to the page bottom and click "Use Support" or contact the athletic department. Please check with your coach/sponsor if you want to check on the status of your current physical date. For new physicals, we will no longer be collecting hard copies of physicals, since parents will be able to take pictures of them and upload those to FinalForms.
Since physicals are good for two years, the athletic department will assist in uploading any current physicals to the system, but we cannot complete this task until each parent creates an account for their student(s). FinalForms also pre-populates information wherever possible, for each of your students, saving you time. FinalForms saves data from season-to-season and year-to-year, meaning that you will never need to enter the same information twice. FinalForms allows users to complete and sign athletic participation forms for your students, including uploading new physicals.
